Field Sick Time

Who is Eligible?

Part-time Field employees who live in a state that requires paid sick leave. Below are the list of states that require Paid Sick Leave or Paid Time Off. If you click on the name of the state, it will pull up a memo regarding the Sick Leave or Time Off policy for that state.

You have to be scheduled for work to use your sick time unless you live in Illinois, Nevada, or Puerto Rico.

**If your state is not listed above then they do not require paid sick time and you do not get paid sick time.

How to Request Paid Leave?

Most states require you to be scheduled for work to receive Paid Sick Leave, see state memos above for details regarding your state sick time law.

You will complete the Field Sick Request Form below. Make sure you list a shift # and select the correct DM. If you do not know your DM, email hr@apolloretail.com. By not listing a shift # or the correct DM, it will delay your request.

Once you submit a request, it is submitted for review and it not approved yet. An email will send to the manager you choose in the drop down for approval. If your manager approves it, it will go to HR for approval. If your manager denies it, you will receive an email stating why it was denied.

Once your manager has approved it, it will go to HR for review and approval. HR will review to make sure that the form is filled out correctly and that you have accrued enough time for what you have requested. If HR approves it, it is approved and will be submitted for processing to be paid out on the pay date that the sick time falls on. If HR denies it, you will receive an email stating why it was denied.

Allow 3-4 business days for review of the sick request by both DM and HR. You will receive email updates once it’s either approved or denied. Please do not continue to email for updates if within the time frame listed above.
