Sr Leads who job title is Site Lead, Site Manager, Site Supervisor, Senior Team Lead, Senior Grocery Lead & Senior Crew Coordinator are eligible for the benefits in the Sr Lead Benefit Guide.
New Hire: You have 30 days from your hire date to enroll in benefits which will be effective the 1st of the month following 30 days. Your hire date counts as Day 1.
Position Change: You have 30 days from your work event to enroll in benefits which will be effective the 1st of the month following the work event.

Dependent Verification

Employees who are currently covering their dependent(s) on their benefit plans will be required to present documentation when enrolling in ADP to verify their dependent’s eligibility in order to remain on the company's insurance plan(s).

Dependents are domestic partners, legal spouses, and then eligible children are defined as biological children, legally adopted children, stepchildren, eligible foster children and children for whom you have legal guardianship or your spouse’s natural born children, and/or a child for whom the court has issued a Qualified Medical Child Support Order (QMCSO) requiring you or your spouse to provide coverage. Children enrollment is covered until the end of the year the child(ren) turns age 26 unless state law states otherwise.

To receive the Health Savings Account Employer Contribution listed in the benefit guide, you have to enroll in the HDHP plan and the Health Savings Account. If you miss a paycheck, you will not receive the Employer contribution on the Health Savings Account (HSA). You must receive a paycheck each payroll to receive the ER contribution. As a reminder, the ER contribution is divided over payrolls.

Keep in mind that once you enroll you will not be able to drop, add or change coverage during 2024/2025 unless you have a qualifying life event.

If you miss four FULL continuous pay periods, your policy will be canceled and you will be offered. If you did not have any payments deducted from your check in the month that your insurance became effective your insurance will be canceled back to 1st of the month and you will not be offered COBRA. Payments have to be deducted from your pay check

**Important Note: The company name on Cigna IDs/documents, will reflect as Channel Partners Group. Channel Partners Group is our parent company. Channel Partners Group will be the name of the Cigna plans.

You will receive physical cards in the mail for Medical and Vision. If you enrolled in Dental, Cigna will no longer mail ID cards for dental, and you can find your dental card at mycigna.com or the mycigna app.
